For those of you who don't know, I am a
CPC (Certified Professional Coder). I work part-time from home. Today while I was working I came across something that made me laugh out loud. As I was reading a
patient's ultrasound record, the technologist noted that the patient was an "extremely poor historian". The rest of the document caries a similar irritated tone as they describe the findings. The moral of the story: If you don't want your medical records to have sarcastic undertones, know your medical history!

That is hilarious. I have been tempted to write things like that when I am charting at work....I don't but I am tempted!
That's awesome. I can't wait to have moments like that. Actually, I did have a similar experience with transcription. At U of M, our thoracic surgeons would do "cash consults." Out of state patients would send in the stack of specified medical records, to see if the patient was a surgical candidate. The docs would dictate a letter to send back to their referring doctor and to the patient. Some of those letters were pretty funny.
